Abstract
[fr] L’insuffisance cardiaque est une préoccupation croissante en santé publique avec le vieillissement de la population et l’amélioration de sa prise en charge. L’augmentation mondiale du nombre d’insuffisants cardiaques et le manque d’organes disponibles encouragent une utilisation croissante d’une assistance ventriculaire gauche « Left Ventricular Assist Device » (LVAD) qui permet de suppléer la fonction cardiaque dans l’attente d’une transplantation. Le LVAD impose des changements complexes et des nouveaux défis aux patients et à leurs aidants proches. Le but de cette étude est d’identifier les besoins psychosociaux et d’envisager des hypothèses d’amélioration de leur prise en charge. Pour cela, nous avons utilisés 5 questionnaires auprès de patients LVAD et d’anciens patients traités par ce dispositif, pris en charge au CHU de Liège, aux Cliniques Universitaires Saint-Luc (UCL) et à l’Hôpital Érasme (ULB). 25 patients traités par LVAD et 17 patients transplantés cardiaques ont été inclus dans cette étude.
Les résultats indiquent que les besoins psychosociaux liés à l’interaction avec les professionnels de la santé et l’obtention d’informations sont considérés par les patients comme les plus importants et les plus satisfaits. L’étude met en évidence les succès de l’équipe soignante et dégage plusieurs pistes de réflexions pour l’amélioration des aides liées aux aspects financiers, au sentiment de culpabilité, de colère, de solitude, ainsi que celles pour faire face à tout changement et à la façon dont les autres me voient.
Cette recherche met en évidence la nécessité d’une prise en charge pluridisciplinaire régulière et suivie du patient LVAD.
[en] Heart failure is a growing public health concern with the aging of the population and the
healthcare improvement. The worldwide increases in the number of heart failure patients and the lack of available organs encourage the usage of left ventricular assist device (LVAD) to supplement the cardiac function while waiting for a bridge to transplant. The LVAD imposes complex changes and new challenges to the patients and their caregivers. The purpose of this study is to identify their psychosocial needs and to consider hypotheses to improve their healthcare management. For this, we used 5 questionnaires with LVAD patients and former patients treated with this device at the University Hospital of Liège, the University Clinics Saint-Luc (UCL) and at the Erasmus Hospital (ULB). 25 patients with LVAD and 17 cardiac transplant patients were included in this study.
The results indicates that the psychosocial needs linked with the interaction with the health
professionals and the obtention of information are considered as the most important and most satisfied. The study highlights the success of the healthcare team and suggests several ideas to improve the help provided with respect to the financial aspect, how to cope with guilt, anger, loneliness and support with change and they way others see me.
This research highlights the needs for regular multidisciplinary care and follow-up of the
LVAD patient.
