Abstract

Introduction : Les inégalités sociales de santé (ISS), en hausse en Belgique et particulièrement en Wallonie, créent des écarts injustes d’accès et d’état de santé entre groupes sociaux. La promotion de la santé offre une réponse adaptée, notamment par divers moyens d’action et stratégies de réduction des ISS. Toutefois, leur prise en compte par les professionnels reste complexe. Les obstacles incluent la nature multidimensionnelle des interventions, l’absence de formations spécifiques, la difficulté d’évaluation et des définitions parfois floues. Comprendre comment ces professionnels intègrent concrètement la lutte contre les ISS dans leurs pratiques est donc essentiel. Matériels et méthodes : Cette recherche a été menée via une étude qualitative exploratoire de type phénoménologique. Elle comprend 12 entretiens semi-directifs auprès de 13 opérateurs de promotion à la santé agréés par l’AVIQ dans le cadre du Plan de Promotion et Prévention à la santé. Cette investigation s’est basée sur le modèle KABP (Knowledge, Attitudes, Beliefs and Practices) afin de comprendre comment les opérateurs de Promotion considèrent les ISS au sein de leurs pratiques. Résultats : L’étude met en évidence que, face à la complexité des définitions et des concepts liés aux ISS, les acteurs de terrain recourent à des stratégies ancrées dans les réalités locales et favorisent la collaboration intersectorielle, tout en adaptant leurs interventions pour en améliorer l’accessibilité et en percevant les ISS comme une occasion de rapprocher les secteurs ; parallèlement, ils amorcent un changement de paradigme vers une approche holistique centrée sur un patient acteur, impliqué de manière étroite et continue dans son parcours. Cette évolution témoigne d’une volonté croissante d’intégrer pleinement la lutte contre les ISS dans les pratiques, mais souligne aussi la nécessité d’un soutien institutionnel et d’outils communs pour pérenniser ces avancées. Conclusion : L’étude révèle un engagement fort des opérateurs à intégrer les ISS malgré une certaine ambiguïté conceptuelle. Ils adaptent leurs pratiques localement, privilégiant une approche holistique et collaborative. Poursuivre les recherches, notamment du point de vue des patients, apparaît essentiel.

Introduction :Social inequalities in health (SIH), which are on the rise in Belgium and particularly in Wallonia, create unfair disparities in access to healthcare and health status between social groups. Health promotion offers an appropriate response, particularly through various means of action and strategies to reduce social inequalities in health. However, it remains complex for professionals to take these into account. Obstacles include the multidimensional nature of interventions, the lack of specific training, the difficulty of evaluation, and sometimes vague definitions. Understanding how these professionals concretely integrate the fight against SIH into their practices is therefore essential. Materials and Methods : This research was conducted as part of an exploratory qualitative study of a phenomenological nature. It includes 12 semi-structured interviews with 13 health promotion operators accredited by the AVIQ as part of the Health Promotion and Prevention Plan. This survey was based on the KABP (Knowledge, Attitudes, Beliefs and Practices) model in order to understand how promotion operators perceive SIH in their practices. Results : The study highlights that, faced with the complexity of definitions and concepts related to SIH, actors in the field resort to strategies rooted in local realities and promote intersectoral collaboration, while adapting their interventions to improve accessibility and perceiving SIH as an opportunity to bring sectors closer together. At the same time, they are initiating a paradigm shift toward a holistic approach centered on patients as active participants who are closely and continuously involved in their care pathways. This evolution reflects a growing desire to fully integrate the fight against SIH into practices, but also highlights the need for institutional support and common tools to sustain these advances. Conclusion : The study reveals a strong commitment on the part of operators to integrate ISS despite some conceptual ambiguity. They are adapting their practices locally, favoring a holistic and collaborative approach. Further research, particularly from the patients' perspective, appears essential.
