Abstract

[en] Mahajanga I is the capital of Boeny region in province of Mahajanga. The weather condition in this place is promoting the mosquito’s development. This result comes from an observational and analytical study in which we could identify the factors influencing vulnerability to malaria in this district. The study covers a timeline period from 2011 to 2018, but we especially choose to work out the cases of 2018 for a survey done with people affected by malaria around the five contaminated places in Mahajanga by refferring to the Heath Care Centers. The sample include 50 random cases and 100 witnesses.
Our approach covers the public, epidemiological and environmental health. The information was collect through :
- some survey within people who are in the same conditions but some are affected by malaria and some are not affected.
-some interview with the resource persons (Public Health, the Sanitation department and beautification service of Mahajanga I)
-the monthly files of malaria archive from 2011 to 2018 in district of Mahajanga I the Health Information System of District of Mahajanga I and from PZaGa Hospital.
-Wearther data of Infoclimat.fr version 5.4 and data on the location of environmental factors (Google Earth), the total number of the population from the year 2011 to 2018 recorded at the statistic service in the Boeny Region.
As far as the results of this Recherch is concerned the climatic conditions is not the main factor of malaria. Other factors like waste management, the environmental no respect of the environment hygiene the economic situation and institutional arragement add on the situation.
Awareess no the identified risk factors can be an essential components in the risk mitigation assessments.

[fr] Mahajanga I est le chef-lieu de la région Boeny de la province de Mahajanga avec sa condition climatique favorable au développement de moustique. Ce travail est un résultat d’une étude observationnelle analytique visant à identifier les facteurs influençant la vulnérabilité face au paludisme de ce district. Il couvre la période comprise entre 2011 et 2018 dont la période 2018 a été incluse pour l’enquête des personnes atteintes du paludisme dans les cinq quartiers les plus contaminés en se référant au centre de santé des bases. La taille de l’échantillon se chiffre à 50 cas et 100 témoins par un tirage aléatoire simple.
Notre approche était à la fois de la santé publique, épidémiologique et environnementale. Les informations ont été recueillies par :
-une enquête auprès des personnes atteintes du paludisme et non atteintes avec une même situation.
-un entretien avec les personnes ressources (Direction de la santé publique et le service d’assainissement et embellissement de Mahajanga I).
-des données archives sur le paludisme mensuel pour les années 2011 - 2018 au service de gestion de système d’information sanitaire de district de Mahajanga I et au CHU PZaGa.
-des données météo Infoclimat.fr version 5.4 et des données sur la localisation des facteurs environnementaux (Google Earth), l’effectif total de la population pour l’année 2011 -2018 au service statistique de la Région Boeny.
Selon les résultats de l’étude, les conditions climatiques n’expliquent pas à elles seules l’occurrence du paludisme. D’autres facteurs interviennent tels que certaines pratiques comme la gestion des déchets, le non-respect d’hygiène de l’environnement, la situation économique et les dispositifs institutionnels.
La prise en considération des facteurs de risques identifiés est une composante essentielle de l’évaluation et de la réduction des risques
