Abstract

[en] Summary
Results-based financing is a model advocated by the World Bank and USAID in Haiti since 1999 in order to improve the performance of the health system. It is expanding worldwide, particularly in Africa, and since 2014, Haiti has been using it on a national scale. The health situation in Haiti reflects the socio-economic situation that the country has been experiencing for some time, with maternal and infant mortality far higher than in other countries in the region, with the Central Department having the highest rate of malnourished children in the country.
In this context, an analysis of the RBF's contribution to the Notre Dame de la Nativité hospital in Belladère shows that it has an impact on the hospital's performance by improving patient satisfaction and the quality of care. Positive effects are observed in some health indicators and the hospital shows an increase in the number of CPN1 and institutional deliveries. A decrease in the number of cesarean patients and a decrease in nosocomial infections were also observed. By acting on these different indicators, Results-Based Financing contributes to performance improvement.

[fr] Le financement basé sur les résultats est un modèle prôné par la banque mondiale et la USAID en Haïti depuis 1999 dans le but d’améliorer la performance du système sanitaire. Il connait une véritable expansion dans le monde particulièrement en Afrique et depuis 2014, Haïti l’utilise à l’échelle nationale. La situation sanitaire en Haïti est pourtant à l’image de la situation socioéconomique que connait le pays depuis quelque temps avec une mortalité maternelle et infantile très supérieure aux autres pays de la région, le département du centre ayant le taux d’enfants malnutris le plus élevé du pays.
Dans ce contexte une analyse des apports du FBR au niveau de l’hôpital Notre Dame de la Nativité de Belladère montre qu’il agit sur la performance de l’hôpital en améliorant la satisfaction des patients et la qualité des soins . Des effets positifs sont constatés au niveau de certains indicateurs de santé et l’hôpital présente une augmentation du nombre de CPN1 et des accouchements institutionnels. Une baisse du nombre de patientes césarisées et une diminution des infections nosocomiales ont été également observés. En agissant sur ces différents indicateurs le Financement basé sur les résultats contribue à l’amélioration de la performance.
