Abstract

[en] Introduction : Chronic pain affects more than a quarter of the Belgian population, compromising patients’ independence and quality of life. In light of this complex clinical condition and the shortage of healthcare personnel, new care models are emerging, such as specialized nursing consultations. This study aims to assess the impact of this intervention about which little is currently known on the subjective experiences of patients receiving pain management care at a suburban hospital in an urban area.
Material and method : A qualitative, exploratory, cross-sectional study was conducted with 13 volunteer patients receiving care in pain management nursing clinics. The study took place at a suburban hospital in an urban area, in connection with the implantation of a neurostimulator. Interviews were conducted until data saturation was reached. These were individual, semi-structured interviews conducted by telephone. They were subjected to thematic analysis based on the framework of Wagner’s Chronic Care Model (CCM).
Results : The interviews reveal a marked contrast between a previous experience often characterised by a series of unsuccessful medical consultations, a lack of understanding and psychological distress, and pain management care that is perceived as well-organised and compassionate. The participants highlight the nurse’s willingness to listen, her availability and the time she devoted to them, as well as the smooth collaboration with the referring doctor. Nursing consultations also promote self-management, helping patients to gradually regain their independence and quality of life.
Conclusion : These findings confirm the value of the Chronic Care Model’s support for self-management and interprofessional coordination, whilst highlighting a relational dimension specific to nursing consultations. The therapeutic alliance that develops in this context appears to compensate for certain perceived limitations of conventional medical care. These findings suggest, with due caution, that this model could be extended to other settings for the management of chronic conditions.
